What failed
An evidence record, coverage fact, inspection option, or destination result did not satisfy the closed evidence contract.
Retryability
Not retryable. Retrying the same value cannot make it valid.
Diagnostics are bounded and do not echo payloads, subjects, or idempotency keys.
Fix
Use a documented role, kind, conclusion, subject, and inspect limit. If the failure came from a destination, update it to return the complete canonical five-role response.
